Job OverviewA Special Needs Coordinator (SNC) is needed to treat and/or support the treatment of patients at a medical facility in Aviano, Italy. The ideal candidate will have experience in care coordination, case management or discharge planning.
Responsibilities
• Use clinical assessment skills to identify sponsors whose family members have special medical and educational needs.
• Collaborate with other key service providers to ensure effective outreach and identification of special needs.
• Independently assess the psychosocial functioning and needs of patients and formulate treatment plans.
• Provide oversight for base-level data entry in AF-provided data management systems.
• Provide clinical training to installation personnel and medical staff.
• Participate in quarterly case reviews and determine appropriate resources for families.
Requirements
• Active, valid, current, and unrestricted license to practice nursing as a registered nurse in any US state/jurisdiction.
• Minimum of two years full-time experience in care coordination, case management or discharge planning experience in a medical setting within the last four years.
• Current Basic Life Support (BLS) certification.
• Knowledgeable in medical privacy and confidentiality (HIPAA); accreditation standards of AAAHC and TJC.
